The top of every cylinder has two holes that let out engine exhaust and let in air. The opening and closing of these holes are synchronized with how the cylinders move and how the camshaft rotates. A worn out or broken timing belt means this movement is not synchronized, causing a buildup of exhaust that is expelled in the form of smoke.
